You may mail your completed enrollment application and premium payment or drop it off at our office.  The Campus Health Insurance Office is on the 2nd floor of the Campus Health Service, which is located between Highland Avenue and the 6th Street Parking Garage.  Please see below our office hours and mailing address.

Benefit information – Please note that once you enroll, you will be automatically reenrolled and billed in future semesters (each fall and spring) unless you cancel your coverage during the published open enrollment periods.
